Once you have Extra Tools, the customised Toolbar will no longer show the ellipsis icon, its place being replaced by one of the Tools from the Extra Tools list.  This is useful but you need to remember the ellipsis won't be there, though it does appear at the head of the drop-down list and is available.

From time to time you may need one of the Tools you've 'parked' in the Extra Tools area.  To access one of these, click and hold for a second on the Tool icon that replaces the ellipsis and the Extra Tools will appear upon a drop-down list.  Click on the one you need. This (Extra) Tool will remain in that position until you chose another Extra Tool. You can also use the method to access the ellipsis Tool and edit the Toolbar

 
 

You can also restore any of the Tools you've 'parked' in the Extra Tools area.  To access these, return to Edit > Toolbar, locate the Tool in the Extra Tools area, drag it to the left side and drop it in place you wish to see it.

Tip:  If you drop it above the blue line which appears it will probably 'nest' with other Tools.  Dropping when the blue line appears below a given Tool should present the selected Tool as a 'stand alone' icon.   If, after use, you choose not to return the Tool to Extra Tools area then it will remain in the new position until you reload the preset when it will return to the Extra Tools area.

If it helps at all, I show below the 'preferred Tools' I have on my Toolbar.  You will notice that I exclude the Dodge, Burn Tools and Sponge as they are all 'destructive' - that is so say, they irrevocably alter pixel values.  There are better methods to perform these functions.

The numbers show Tools relate to the customised list and how Tools are 'nested'. For example #7 shows the Healing style Tools within the set. Some Tools are 'stand alone' and don't have any subsidiaries.
